EVERY YEAR IT IS OUR PRIVILEGE to ask the region’s physicians to put forward the peers they believe are stellar in their field. This year, 635 doctors have been recognized with this honor in 126 specialties, from addiction medicine to weight management and everything in between.
But this is not an easy time to be a medical professional. With vaccine deniers, federal funding cuts to medical research, high rates of workplace violence, burn out, and student debt, this once most respected of careers has become something of a battleground. In “The Weight of the White Coat,” Baltimore contributor Dan Cook asks physicians the burning question: Why would anyone want to be a doctor now? Through interviews with young physicians fresh out of medical school and those more seasoned, the story explores medicine’s challenges—and the ways it continues to be rewarding.

WHO OVERSEES THE LIST? In addition to our editorial staff, winning doctors are fact-checked by staff to ensure they are licensed and in good standing, and our panel of physician advisors provides advice and oversight. (Advisers are not eligible to be included on the list in the year they serve.)

Our Advisors
Linda Greif Adler, M.D., F.H.M., is the chief of the division of hospital medicine at University of Maryland (UM) St. Joseph Medical Center and has served as past president of the UM St. Joseph medical staff. Dr. Adler earned her medical degree from The Johns Hopkins University School of Medicine in 2000 and subsequently completed her internship and residency at The Johns Hopkins Hospital. She is board-certified by the American Board of Internal Medicine and is a Fellow of the Society of Hospital Medicine.
Melvin S. Blanchard, M.D., M.A.C.P., is the chairman of medicine at GBMC and adjunct professor of medicine at the University of Maryland. He was professor of medicine, vice chair for Medical Education in the Department of Medicine, and chief of General Medicine at Washington University in St. Louis. He was also chairman of the board for the Alliance for Academic Internal Medicine (AAIM). Dr. Blanchard earned his Doctor of Medicine degree from University of Tennessee Health Science Center and completed his residency in internal medicine at Barnes-Jewish Hospital/Washington University.
Carole Fakhry, M.D., M.P.H., is a head and neck surgical oncologist at Johns Hopkins (JH). She received her medical degree from JH and a master’s in public health from JH Bloomberg School of Public Health. She is a professor, serves as associate dean of clinical affairs for JH School of Medicine, chief of the division of Head and Neck Surgery in the Department of Otolaryngology Head and Neck Surgery, director of the Head and Neck Cancer Center, and director of the head and neck group in the Bloomberg-Kimmel Institute for Cancer Immunotherapy.
Samuel Smith, M.D., is vice president physician advisor for LifeBridge Health, having recently retired as chief medical officer for Northwest Hospital. Prior to assuming that role, he served as the chief quality officer and chair of the Department of Gynecology for the hospital. Smith earned his medical degree from the University of Maryland School of Medicine, completed his residency at Georgetown University Medical Center, and completed his fellowship in reproductive endocrinology-infertility at Brigham and Women’s Hospital, Harvard Medical School. Smith is board-certified in obstetrics and gynecology and reproductive endocrinology-infertility.
Jennifer Taylor, M.D., earned her medical degree at State University of New York at Buffalo and completed her residency in OB-GYN at the University of Maryland School of Medicine. She now practices at Mercy Medical Center. Board-certified in obstetrics and gynecology, she has participated in several research initiatives and has numerous honors, publications, and presentations to her credit. She is a fellow of the American College of OB-GYN, and a member of the American Medical Association and the Douglass Obstetrics and Gynecology Society.

OTOLARYNGOLOGY
WHAT MADE YOU CHOOSE THIS SPECIALTY? The vast array of both medical and surgical conditions that are cared for by otolaryngologists is what attracted me. In addition, I find appealing some of the key research areas we investigate in otolaryngology, particularly those relating to the cure for cancer. And as someone who values the sacredness of the patient-doctor relationship, I am gratified that otolaryngologists are able to have longitudinal relationships with their patients that last many years. Finally, as the majority of my patients are diagnosed with head and neck cancer, providing comfort and hope to patients with this condition is something that gives me deep fulfillment and satisfaction.
WHAT DO YOU ENJOY MOST ABOUT THIS WORK? Curing patients of their cancer is very important to me, but I most enjoy patients returning to their full humanity, pursuing their passions and hobbies as well as enjoying friends and family. The stories that they share during their visits affirming their return to form absolutely fill me up and inspire me.

WHAT MADE YOU CHOOSE THIS SPECIALTY? The intellectual curiosity and the fact that nephrologists [a doctor who specializes in kidney disease] get to specialize while still remaining proficient in internal medicine. Nephrology requires both deep expertise and broad medical knowledge. What’s unique is that we work heavily both inpatient and outpatient, which allows us to establish real longitudinal relationships with our patients.
DO YOU HAVE A STORY THAT UNDERSCORES WHAT YOU LOVE ABOUT THIS JOB? I took over the care of a patient with kidney stones. Her calcium levels were borderline high, initially attributed to one of her medications. Despite stopping the medication, her calcium levels remained borderline high, and her parathyroid hormone level was inappropriately normal, which suggested primary hyperparathyroidism.
After coordinating with surgery for parathyroid adenoma removal, I saw her months later via video and she had a big “Thank You” message written on colored paper. She told me that symptoms like mild anxiety and restlessness had also resolved, and she didn’t even know they were related to her calcium and stone problems. Seeing that transformation and knowing that careful attention to detail can make a difference in someone’s life, that’s what drives my passion for nephrology.

INTERVENTIONAL RADIOLOGY
WHAT IS SOMETHING NON-PHYSICIANS MIGHT BE SURPRISED TO LEARN ABOUT INTERVENTIONAL RADIOLOGY (IR)? I suspect it would be that IR exists in medicine. We are a small field that is mostly hospital-based, so many patients don’t know who we are. We do a wide variety of procedures throughout the human body that all have one thing in common: They can be done through a needle or a small catheter inserted through a tiny incision that can be closed with a Band-Aid. We do many types of biopsies, treatment of tumors using intense heat or cold delivered through needles, delivering chemotherapy through catheters directly into tumors, draining abscesses with small catheters, draining blockages in the biliary or urinary systems if they cannot be treated with conventional surgical procedures, and many other procedures.
HOW DO YOU UNWIND OUTSIDE OF THE OFFICE? Gardening and working on cars. We’re eating something from the garden almost yearround, between fresh harvests and careful storage of the harvest like winter squash that keeps well for many months in a cool dry place. There’s always work to do in the garden.

Welding really is a very interesting process that involves a lot of physics and chemistry and using your hands. When it’s done well, it’s almost artwork. Now, I’m always looking for something to weld. If I had taken welding lessons after high school or in college, I might not have gone to medical school.

WHAT ARE SOME MAJOR FACTORS AFFECTING YOUTH MENTAL HEALTH TODAY? There’s a lot going on in kids’ lives these days. Family stress, school pressure, social media, and even bigger issues like community violence or financial hardship can all take a toll. The pandemic added even more layers——like isolation and disrupted routines.
HOW CAN A PARENT OR GUARDIAN TELL WHEN IT’S TIME TO REACH OUT FOR HELP? If a child’s mood or behavior changes and it lasts for a few weeks, starts affecting their daily life, or causes stress at home or school, it’s a good idea to talk to a specialist. Things like pulling away from friends, being unusually irritable or anxious, trouble sleeping, falling grades, or saying things that sound hopeless are all signs to watch for.
WHAT CAN WE ALL DO TO SUPPORT YOUTH MENTAL HEALTH? It starts with creating spaces where kids feel safe to talk and be themselves. Listening without judgment, showing empathy, and encouraging open conversations are key. Schools, families, and communities can help by talking more about mental health and breaking down the stigma around asking for help. And we all have a role to play in making care more accessible and inclusive.
WHAT’S THE MOST REWARDING PART OF YOUR WORK? Seeing a young person start to feel hopeful again is incredibly meaningful. Helping families reconnect and heal is just as powerful. Every time a child feels truly understood and supported, I’m reminded why I do this work.

WHAT MADE YOU CHOOSE THIS SPECIALTY? My journey into cardiac surgery began long before medical school, at my mother’s side in the cardiac intensive care unit where she worked as a nurse. I was captivated by the idea that we could stop a human heart, repair it with precision, and bring it back to life. To this day, it feels like one of the great miracles of medicine.
WHAT DO YOU FIND MOST REWARDING IN THIS JOB? The most rewarding part for me is the immediacy of our work; there’s nothing abstract about saving a failing heart. You see the problem, you fix it, and you watch someone’s future open before them.
WHAT IS SOMETHING NON-PHYSICIANS MIGHT BE SURPRISED TO LEARN ABOUT CARDIAC SURGERY? Most people imagine a lone surgeon operating on a heart, but in the operating room alone there are at least seven highly trained individuals . . . all working in perfect coordination.
ANOTHER SURPRISING TRUTH: To operate on the heart, we actually stop it. We use advanced technology to take over its life-sustaining work, giving us the opportunity to repair arteries or valves. Then, we restart it. What’s even more surprising is that even though I held their heart in my hand less than 24 hours ago, most times patients are sitting in a chair the very next morning.

WHAT MADE YOU CHOOSE THIS SPECIALTY? I chose pediatrics because this is what I’ve always wanted to do. My parents tell the story that I came home from preschool one day and said that I wanted to be a “baby doctor,” and that never changed.
WHAT’S THE MOST CHALLENGING PART OF YOUR WORK? The most challenging aspect of working with children is having to give a parent bad news about their child’s health. That really affects me in a profound way. It’s something you never forget. Thankfully, in general pediatrics, it doesn’t happen too often, but even one is too many.
WHAT IS MOST REWARDING? Each day I can work with young people is the reward. Seeing not only the patients grow but seeing the parents and guardians grow as their caregivers is very rewarding. I do love telling the stories of a few of my patients who have endured illnesses such as severe seizures leading to coma and others who have had cancer. They have all survived and are thriving. I call them “my miracle babies.”
WHAT ARE YOUR TOP THREE TIPS FOR PARENTS/GUARDIANS TO KEEP THEIR KIDS HEALTHY? Give your child a healthy diet, encourage good sleep routines, and vaccinate to help prevent disease.
